ACL Surgery Recovery Timeline: What Each Phase Really Looks Like

From crutches to return-to-sport testing — with honest dates

Updated July 20262 minute read

ACL reconstruction recovery runs 9–12 months, not the 6 many people expect. Early weeks focus on swelling, extension, and quad activation; months 3–6 build strength and running; return to cutting sports should wait for passed strength and hop testing at 9+ months, because earlier return sharply raises re-tear risk.

How long does ACL surgery recovery really take?

Plan on 9–12 months to return to cutting and pivoting sports, and about 3–4 months before running feels like part of life again. That's longer than most people expect — and the single most useful thing you can do is accept the long timeline early instead of fighting it at month 6.

One caveat before the phases: your surgeon's protocol wins. Timelines shift with graft type, meniscus repairs done at the same time, and surgeon preference. This guide shows the typical shape of recovery so nothing surprises you.

Weeks 0–2: calm it down

The jobs are unglamorous: control swelling, get the knee fully straight, and wake the quad up. Expect crutches, a lot of ice and elevation, and exercises that look trivial but aren't — quad sets and heel slides are the foundation everything else stands on. Full extension equal to your other knee is the milestone that matters most this phase.

Weeks 2–6: walk like yourself again

Crutches phase out as the quad returns and your gait normalizes. Range of motion pushes toward full bend, and strength work starts light: leg presses, step work, balance. A knee that's still swelling after sessions is telling you the dose is too high — that feedback loop is the skill to learn now.

Weeks 6–12: build the engine

This is the strength block — squats, lunges, single-leg work, hamstring loading — progressed steadily. The knee usually feels close to normal here, which is where people get in trouble: the graft is actually at a vulnerable stage of remodeling. Feeling good is not the same as being ready.

Months 3–6: running and the long middle

Running typically starts around the 3–4 month mark — but the best programs treat it as a test to pass, not a date to reach: quad strength approaching the other side, good single-leg control, a calm knee. In reviews of ACL protocols, most cleared running around 12 weeks, but fewer than one in five used strength or performance criteria — be one of the people who does.

Months 6–9+: earn the return

Return to cutting sports should be gated by testing — quad strength and hop tests compared to your other leg — at 9 months or later. This isn't caution for its own sake: in the Delaware-Oslo cohort, re-injury rate dropped roughly 51% for each month return was delayed up to 9 months, and athletes who passed return criteria re-injured at 5.6% versus 38.2% for those who didn't. Ask your PT to test you, not just clear you.

When to call your surgeon instead of pushing through

  • A hot, increasingly swollen knee with fever — possible infection
  • Calf swelling, warmth, or tenderness — possible DVT, urgent
  • A sudden pop, new giving-way, or a knee that locks solid

The money side

A 9-month rehab is a real investment. Cash-pay PT prices are covered in how much PT costs — comparing total plans of care, not per-visit prices, matters most on long rehabs. For the injury itself, start with the ACL tear guide, and when running is on the horizon, use return to running after injury to build the progression.

Frequently asked questions

Most people wean off crutches within 1–3 weeks, once the quad is firing well and walking looks normal. Rushing this trades a limp you keep for months — let your PT clear you.
