What's the difference between cash-pay and insurance-based physical therapy?

The two models are built around very different things. Insurance-based PT is built around billing codes and reimbursement. To stay viable on what insurers pay per visit, many clinics see several patients at the same time, hand parts of your session to aides or assistants, and shape your plan of care around what your plan will approve and how many visits it will cover. You may also deal with copays, deductibles, prior authorizations, and surprise bills that don't show up until weeks later. Cash-pay PT is built around you. Cash-pay (also called out-of-network, private-pay, or self-pay) means you pay your therapist directly, so there's no insurance company dictating your treatment. In practice, that usually means: One-on-one time with a licensed PT for your full session — not shared attention or handoffs to an aide. A plan built around your goals, not around billing codes or visit caps. Transparent pricing you see before you book — no surprise bills. Faster access — you can start care without waiting on insurance authorizations. Because sessions tend to be longer and more focused, many people reach their goals in fewer total visits.

Value more one-on-one time and a plan driven by your recovery, not your insurer. Want price transparency and the ability to start care right away. To be straightforward: cash-pay isn't automatically the right call for everyone. If you have rich, low-deductible coverage with strong in-network options, using insurance may cost you less out of pocket. The point of RightPT is to put the real numbers and choices in front of you so you can decide.

The most useful comparison isn't price per visit; it's the total cost to actually reach your goals.

In most cases, yes. Physical therapy for an injury, pain, a movement problem, recovery, or a medical condition is generally an eligible HSA/FSA expense, and many people pay with pre-tax dollars. Card acceptance and reimbursement rules can vary by HSA/FSA administrator, so confirm with your plan if you're unsure.

After your visit, you can get an itemized superbill that includes everything your insurer needs to consider out-of-network reimbursement: your details, the provider's information (NPI, tax ID, and license), the date of service, the diagnosis (ICD-10) and procedure (CPT) codes, and the amount you paid. Here's how it works: you pay your provider directly, then submit your superbill to your insurance company. If your plan includes out-of-network benefits, you may be reimbursed for part of the cost — commonly a percentage after you've met your out-of-network deductible. Reimbursement isn't guaranteed and depends on your specific plan, so it's worth calling the number on your insurance card to ask about your out-of-network PT benefits before you book.

What if I have Medicare or Medicaid?

Separately, some insurance plans may still require a referral to reimburse you, even where direct access is allowed. Your provider can help explain what applies to your situation.

What happens at my first appointment?

Your first appointment on RightPT is an Initial Evaluation — a dedicated, one-on-one session with your physical therapist that usually runs 45–60 minutes (some run longer). It typically moves through four stages: Your story. Your PT asks about your symptoms — where it hurts, how severe it is, and when it started — along with your history of injuries, surgeries, or conditions, anything you've already tried, and what you want to get back to. A hands-on assessment. Your PT evaluates the things that point to the root cause: range of motion, strength, flexibility, posture, balance, and how you actually move (walking, squatting, reaching, or the specific motions tied to your goals), plus targeted clinical tests. They'll explain each test as they go, and nothing should be sharply painful — speak up if anything is. Findings and a plan. Your PT explains what they found and what's driving it (a physical therapy diagnosis, which can differ from a doctor's diagnosis), then builds a treatment plan around your goals with a realistic timeline so you know what progress should look like. Treatment, often the same day. Because cash-pay visits give you the therapist's full attention, many people start treatment on day one — hands-on care, your first exercises, and education — and leave with a home program and clear next steps. To get the most out of it: complete any intake forms before you arrive, and wear comfortable, athletic-style clothing you can move in that gives easy access to the area you're treating (skip jeans and restrictive layers).

RightPT shows two kinds of profiles, so you can both discover therapists near you and book with confidence: Verified providers have joined RightPT and been reviewed for active licensure and profile accuracy. They've confirmed their own details — pricing, availability, and services — carry the Verified on RightPT badge, and can be booked instantly through the platform. Directory listings are therapists we've added so you can find more providers in your area who aren't on RightPT yet. Because the therapist hasn't confirmed these yet, some details may be incomplete or out of date, and you can't book them directly until they join. Found a directory listing you'd like to see? Just tell us, and we'll personally reach out to bring that therapist onto RightPT so you can book them. Either way, every booking on RightPT is with a verified provider.
