A deductible is the amount you pay out of pocket each year before your insurance plan starts sharing costs. Most plans track separate — and usually higher — deductibles for out-of-network care.
Until your deductible is met, claims you submit get "applied to the deductible" — no money back yet, but every allowed dollar counts toward the threshold. Once it's met, the plan starts paying its percentage (coinsurance) of the allowed amount.
For cash-pay PT patients, the number to know is the out-of-network deductible — often separate from and higher than the in-network one. Find it in your benefits summary, then see how it flows through a real reimbursement example.
