Upper Back Pain When Twisting? Here's What It Means and What to Do

Upper back pain that flares when you twist or rotate usually means a joint or rib in the upper back is irritated — twisting loads the facet joints and rib connections. It can also be a muscle strain reacting to rotation. It's typically not serious and improves with gentle mobility, posture variety, and strengthening. Sharp pain with deep breathing, or any chest or neurological warning signs, should be checked.

What this usually means

Most likely causes. Thoracic facet joint irritation and rib joint dysfunction commonly flare with twisting; a muscle strain can too. Less common causes. Persistent or severe rotational pain, or pain with breathing or neurological symptoms, warrants assessment.

What you can try at home

Keep the upper back gently moving — including comfortable rotation — rather than guarding it, which stiffens things further. Reduce the specific aggravating twists temporarily, use warmth for muscle tension, and add gentle thoracic mobility. Strengthening builds tolerance.
