TFCC Wrist Injury (Pinky-Side Wrist Pain): Symptoms, Causes & PT Treatment

The TFCC (triangular fibrocartilage complex) is a cushion-and-ligament structure on the pinky side of the wrist that helps it bear load and rotate. A TFCC injury causes pain on the pinky side, often with twisting (turning a doorknob), gripping, or weight-bearing, sometimes with clicking. Many TFCC injuries are managed well with relative rest, splinting, and progressive strengthening; some need further evaluation.
What is a TFCC injury?
The TFCC stabilizes the pinky side of the wrist and the forearm rotation joint. It can be injured acutely (a fall or forceful twist) or wear gradually.
Symptoms
Common features are pain on the pinky side of the wrist, worse with twisting, gripping, or pushing through the hand, sometimes with clicking or a sense of instability.

A fall onto an outstretched hand, forceful twisting, repetitive loading (racket sports, gymnastics), and individual wrist anatomy contribute.
How it's diagnosed
A clinician examines the pinky-side wrist with specific tests; imaging (including MRI) is used in persistent or significant cases.
Physical therapy treatment
