Snapping Hip Syndrome: Symptoms, Causes & PT Treatment

Updated July 20261 minute read

Snapping hip is a snapping or popping sensation when you move the hip, caused by a tendon gliding over a bony bump — at the outer hip or the front of the hip. When it's painless, it's usually harmless and just needs better hip control and strength. When it's painful or comes with catching, the labrum or a tendon may be irritated and it's worth a physical therapy assessment.

What is snapping hip syndrome?

It describes an audible or felt snap during hip movement. The two common types are external (a band snapping over the outer hip bone) and internal (a hip flexor tendon snapping at the front).

Symptoms

Common features are a snap or pop with walking, rising from a chair, or rotating the hip. It may be painless or accompanied by tightness; pain or catching suggests added tissue irritation.

What causes it / risk factors

Tendon tightness, repetitive hip movement (common in dancers and runners), muscle imbalances, and sometimes joint or labral issues.

How it's diagnosed

Diagnosis is clinical by reproducing the snap and locating its source. Imaging is reserved for painful cases or suspected labral involvement.

Physical therapy treatment

What a PT does. Your therapist identifies the snapping source, improves hip control and flexibility, and addresses any underlying irritation. Exercises & progression (overview). Hip strengthening, mobility, and movement retraining to reduce the snap and any pain. Recovery timeline. Painless snapping often improves quickly with control work; painful cases take longer.

Treatment options compared

PT is first-line. Painless snapping rarely needs anything more. Surgery is uncommon and reserved for persistent painful cases.

Red flags

Seek care for painful catching/locking, inability to bear weight after injury, or a hot feverish joint.
