Rib "Out of Place" (Rib Joint Dysfunction): Symptoms, Causes & PT Treatment

A rib that feels "out of place" is usually an irritated rib joint where the rib meets the spine — not literally dislocated. It causes sharp, localized upper-back or side pain, often worse with deep breathing, coughing, or twisting. It's typically not serious and responds well to physical therapy (mobility, breathing, and strengthening). Because deep-breathing pain can rarely signal a heart or lung problem, warning signs should always be ruled out.

What is rib dysfunction?

It describes irritation of a rib's joint connections near the spine, producing localized pain. Despite the "out of place" feeling, the rib isn't truly dislocated.

Symptoms

Common features are a sharp, well-localized pain in the upper back or along a rib, worse with deep breathing, coughing, twisting, or certain positions, sometimes with a tender spot.
