Biceps Tendon Pain at the Elbow (Distal Biceps Tendinopathy): Symptoms, Causes & PT Treatment

Distal biceps tendinopathy is overload of the biceps tendon where it attaches at the front of the elbow, causing pain there with lifting, pulling, and palm-up forearm rotation. It's an irritated tendon and responds to load management and progressive strengthening. The crucial exception: a sudden pop at the front of the elbow while lifting, with weakness, bruising, and a change in the muscle's shape, can mean a tendon rupture — seek prompt evaluation, as repair is often time-sensitive.

What is distal biceps tendinopathy?

It's an overuse irritation of the biceps tendon at its elbow attachment, which loads heavily during lifting and forearm rotation.

Symptoms

Common features are pain at the front of the elbow, worse with lifting, pulling, or turning the palm up against resistance, sometimes with tenderness and mild weakness.

Repetitive or heavy lifting and pulling, a sudden increase in load, and age-related tendon changes contribute.

How it's diagnosed

Diagnosis is clinical from the front-elbow location and aggravating tasks. Imaging is used if a tear is suspected.
